Notice of Data Security Incident Involving TriZetto Provider Solutions (TPS)
Our organization is committed to protecting the privacy and security of our patients’ information. We are posting this notice to inform the community about a data security incident involving TriZetto Provider Solutions (TPS), a vendor that provides insurance eligibility verification services used by many healthcare organizations nationwide.
Although this incident did not involve our internal systems, some of our patients’ information was involved through TPS. This notice explains what happened, what information was affected, and what patients can do.
What Happened
On October 2, 2025, TPS detected suspicious activity within a web portal used by certain healthcare providers to access its systems. TPS immediately began an investigation, engaged external cybersecurity experts, and notified law enforcement.
After completing a detailed review, TPS confirmed that an unauthorized party accessed certain insurance eligibility verification records beginning in November 2024. TPS began notifying affected providers, including our organization, on December 9, 2025.
What Information Was Involved
The information involved varied by individual and may have included:
- Name
- Address
- Date of birth
- Social Security number
- Health insurance member number (including Medicare identifiers)
- Health insurer name
- Provider name
- Primary insured or dependent information
- Other demographic or health insurancerelated details
No financial account, banking, or credit card information was involved.
What TPS Has Done
TPS has taken the following steps:
- Secured the affected system and eliminated the unauthorized access
- Conducted a comprehensive investigation with industryleading cybersecurity experts
- Reported the incident to law enforcement
- Implemented enhanced security measures to help prevent future incidents
TPS is also offering free identity monitoring services to affected individuals, including:
- Singlebureau credit monitoring
- Singlebureau credit report
- Singlebureau credit score
- Proactive fraud assistance
Enrollment in these services will begin on February 9, 2026, allowing time for TPS to finalize processing of affected patient information.
What Our Organization Is Doing
We are working closely with TPS to ensure appropriate notification and support for impacted individuals. Affected patients will receive a mailed letter with details about the incident and instructions for enrolling in complimentary identity monitoring services.
We encourage patients to carefully review their letter when it arrives and follow the steps provided.

- Enrolling in the free identity monitoring services provided
- Reviewing account and credit report activity for anything unusual
- Contacting your financial institutions if you notice suspicious activity

Organizata jone eshte e perkushtuar per te mbrojtur privacine dhe sigurine e informacineve te pacienteve tane. Ne jemi duke e publikuar kete njoftim te informojme komunitetin ne lidhje me nje incident te sigurise te te dhenave duke perfshire Trizzeto Provider Solution (TPS) nje shites qe ofron sherbime verifikimi te pershtateshmerise se sigurise te perdorura nga shume organizata te kujdesit shendetesor ne te gjithe vendin.
Edhe pse ky system nuk perfshin sistemet tona te brendeshme, disa nga informacionet e pacienteve tane ishin perfshire nepermjet TPS. Ky njoftim shpjegon cfare ndodhi, cfare informacione ishin prekur, dhe cfare pacientet mund te bejne.

Ne dt 2 Tetor, 2025, TPS zbuloi nje aktivitet te dyshimte brenda nje portali ne internet te perdorur nga disa ofrues te kujdesit shendetit per te hyre ne system. TPS filloi menjeher nje hetim duke angazhuar eksperte te jashtem te sigurise kibernetike, dhe njoftoi forcat ligjore.
Pas perfundimit te nje shqyrtimi te detajuar, TPS kofirmoi qe nje pale e paautorizuar kishte akses ne dis ate dhena te verifikimit te pershtatshmerise se sigurimit duke filluar qe nga Nendori I 2024. TPS filloi te njoftoje mjeket, duke perfshire organizaten tone, ne dt 9 Djetor 2025.
Cfare Informacioni Ishte Perfshire
Informacioni i perfshire ndryshonte ne varesi te individid dhe mund te kete perfshire:
- Emrin
- Adresen
- Dite lindjen
- Numrin e Social Security (numrin sigurime shoqerore )
- Numrin e siguracionit shendetesor duke (perfshire identifikuesit e Medicare)
- Emrin e siguruesit Shendetesor
- Emrin e doktorit( ofruesit te sherbimit shendetesor)
- Informacion mbi te siguruarin kryesor ose personin ne varesi
- Detaje te tjera demografike ose te lidhura me sigurimin shendetesor.
Nuk ishte perfshire asnje llogari financiare, bankare apo informacion kredi karte .
Cfare ka bere TPS
TPS ka ndermarre hapat e meposhtem :
- Ka siguruar systemin e prekur dhe eleminoi akseset e paautorizuara.
- Kryem nje hetim gjitheperfshires me eksperte te industries kibernetike
- Raportuam incidentin te forcat e ligjit
- Zbatuam masat te perforcuara sigurie per te ndihmuar ne parandalimin e incidenteve te ardhshme.
TPS gjithashtu ofron sherbime falas te moniturimit per individed e prekur, duke perfshire:
- Monitorim te kreditit te nje zyre te vetme
- Raport te kreditit ten je zyre te vetme
- Pike krediti te nje zyre te vetme
- Ndihme proactive ndaj mashtrimit.
Regjistrimi ne keto sherbime do te filloje ne dt 9 Shkurt, 2026, duke I dhene kohe TPS te finalizoje perpunimin e informacioneve per pacientin e prekur.
